The practice covers Wythall, Hollywood and the surrounding area. If you are unsure whether your address is in our catchment, please ask one of our receptionists.
If you move outside of the area, you should register with another doctor immediately.

It takes around 15 minutes to complete a registration. You need to complete the process in one session. If you are registering more than one person make sure to give yourself enough time as you will need to complete a registration for each person before submitting them all at once. We aim to register you within 5 days of receiving your details.
All patients wishing to join the practice will be offered an appointment with the practice nurse upon acceptance of your registration. This enables us to obtain a brief medical history until your medical records are received from your previous GP. It is important that you make and keep this appointment.
If you are unable to utilise the link then please download the appropriate form, fill it out and bring it along with you to the surgery when you come to register. There will be other documentation for you to complete when you attend.
