We are currently promoting, Pnuemo and Shingles vaccines in conjunction with the Men ACWY and childhood and travel vaccinations.  Please contact the surgery if you require further information.

STATEMENT OF INTENT

Practice Plan for Patient Online Services

The Practice plans to offer the facility for patients to view online, export or print detailed coded information held in their own records from 28th March 2016.  These dates are subject to necessary NHS GP systems and software being available to the practice.  We currently offer Appointment Booking, Repeat Prescription Requests and Allergy information.

 

 

New Patient Registration

registrationAll patients wishing to join the practice are required to complete a registration form and need to make an appointment to see a nurse when registering. This enables us to obtain a brief medical history until your medical records are received from your previous GP. It is important that you make and keep this appointment.

Please download the appropriate form, fill it out and bring it along with you to the surgery when you come to register. 

A registration pack will be provided when you attend the surgery.

Practice Area

The practice covers Wythall, Hollywood and the surrounding area. If you are unsure whether your address is in our catchment, please ask one of our receptionists.

If you move outside of the area, you should register with another doctor immediately.
